General Information

Founded by Physicians

In 1984, Capital District physicians collaborated to launch CDPHP, a not-for-profit individual practice association (IPA) model health maintenance organization (HMO). More than 25 years later, CDPHP is still guided by its valued physician base and has evolved into an award-winning health benefits provider offering a variety of commercial and government-sponsored plans to the diverse communities it serves.

CDPHP continues to be a physician-guided plan today. A 15-member board of directors, including eight physicians, governs CDPHP, along with community directors who play a vital role in policy setting and administration.

 

Lines of Business

While the CDPHP practitioner network includes more than 10,000 providers and practitioners, we also offer an extensive national network with access to top providers across the country. The CDPHP family of products includes three lines of business:

  • Capital District Physicians' Health Plan, Inc. (CDPHP®) - HMO, Healthy New York, Medicare Choices (HMO), Medicaid, Child Health Plus, and Family Health Plus.
  • CDPHP Universal Benefits,® Inc. (CDPHP UBI) - Preferred Provider Organization (PPO), High Deductible PPO, Medicare Choices (PPO), Exclusive Provider Organization (EPO), and High Deductible EPO products.
  • Capital District Physicians' Healthcare Network, Inc. (CDPHN) - Administrative services organization (ASO) and self-insured plans.

1 The service area is a particular geographic region where the plan accepts members. Within the CDPHP service area, members may enroll through an employer-sponsored plan, a chamber-sponsored plan, a government program, or an individual product.
